Outstanding Public Health Advocate Nomination Form

  • The Outstanding Public Health Advocate Award was established by the Wyoming Public Health Association to honor an individual who has made significant contributions to public health in the State of Wyoming. This award had previously been called the Wyoming Politician Award and was limited to elected officials. The Wyoming Public Health Association expanded the award’s scope in 2006 to include ANY person who has made significant and lasting contributions to public health in Wyoming, whether or not the individual is a resident of Wyoming.

    AWARD CRITERIA:
    Nominee need not be a member of the Wyoming Public Health Association. Nominee must have distinguished him/herself by making a significant contribution to public health in Wyoming in at least one of the following ways:
    • Demonstrated significant leadership and/or mentoring skills in public health issues;
    • Contributed to the promotion and protection of the health and well being of the citizens of Wyoming;
    • Devoted time and energy to advocating for public health either in an official or volunteer capacity;
    • Exhibited understanding and support for principles of public health as a human right;
    • Worked to secure optimal community and public health.
